What is All-on-4?
All-on-4 is a treatment concept that replaces a complete arch of teeth with a fixed bridge supported by only four dental implants. The posterior implants are placed at an angle, which in most cases avoids extensive bone grafting and allows a full arch to be supported with fewer implants.
Before surgery we use 3D CBCT, an intraoral scanner and clinical photographs to plan the implant positions and the shape, size and position of the new teeth, and prepare a surgical guide for the day of the procedure. This gives each patient a personalised plan and places the implants in the optimal position and angle for stability.
Advantages of All-on-4
- One surgical phase – failing teeth and inflamed tissue are removed and all implants are placed in a single procedure.
- Immediate teeth – fixed provisional teeth are fitted on the implants within hours of surgery or within a few days at the latest.
- Fast recovery – extensive bone reconstructions and additional procedures can usually be avoided.
- Cost-effective – a full arch on four implants is often less expensive than conventional implant techniques.
- Better speech and chewing – especially noticeable in patients who previously had very loose teeth or dentures.
All-on-4 or All-on-6?
All-on-4 is an excellent solution for many patients, but some situations need more than four implants. All-on-6 uses six implants and gives the bridge even more support, which is important with more extensive tooth loss or poorer bone quality. Six implants increase the cost of treatment. Which option is right for you is decided on the basis of the examination and 3D CBCT.
Why tissue adaptation matters
All-on-4 and All-on-6 are not just about placing four or six implants. Adapting the bone and soft tissue is essential to create enough space for the final teeth and to make cleaning and maintenance easy. Because this step is more demanding than implant placement itself, it is often skipped – which increases the risk of problems with the teeth or the implants later. We plan the final tooth position digitally and prepare the tissue adaptation before you arrive for surgery.
Treatment timeline
- Consultation and planning – examination, 3D CBCT, intraoral scan, photographs and a written cost estimate.
- Surgery day – removal of failing teeth, implant placement, tissue adaptation and bone regeneration, and fitting of immediate fixed teeth.
- Permanent teeth in 1–2 weeks – aesthetic, functional teeth with a rigid titanium framework that also supports implant healing.
- Optional upgrade – after 6 months or more, the teeth can be upgraded to zirconia or ceramic.
Because the surgical phase and the permanent teeth are completed within about two weeks, this timeline also works for patients travelling from abroad.
